  • Do I have out-of-network benefits for chiropractic / acupuncture / massage?

  • What is my out-of-network deductible, and is it met?

  • What is my coinsurance after the deductible?

  • Do I have visit limits, referral requirements, or pre-authorization requirements?

  • What is the reimbursement rate for out-of-network services?

  • How do I submit claims (mailing address or online portal)?

  • Ask for a reference number for the call.
